Core services| HIH is a short term in home program specifically targeting Veterans with acute or subacute medical issues related to: CHF, COPD, Pneumonia, Cellulitis, UTIs, Wound Care, and IV Antibiotics. Most patients will receive 10 home visits. Exceptions may be made for selected CHF patients and those requiring long term antibiotics, who could stay in the program up to 30 days.

Area served| Rhode Island, Connecticut, and Massachusetts – all within a 30 mile radius of the Providence VA Medical Center
Outcomes or successes| Since July 2019, we have reduced the 30 day readmission rate from over 30% to 16%.
Unique feature| The HIH program is unique compared to typical visiting nurse home services because its goals are to reduce hospital admissions, reduce hospital readmissions, reduce cost of care, increase patient satisfaction, improve continuity of care, facilitate communication to VA providers, patient centric care, and shared decision making.
