Unique feature| Through advanced care at home, patients will transition to a home setting and receive compassionate, high-quality care and recovery services under the direction of Mayo Clinic providers. This care model is built on three primary pillars:

  • A Mayo Clinic physician-led 24/7 virtual monitoring center that provides patients with around-the-clock access to their care team
  • On-demand, acute, medical care management
  • Rapid-response teams to deliver services and supplies
