Monitoring your hospital at home program’s performance and integrity is central to its short- and long-term sustainability. Reporting on key metrics, for example around escalation and mortality, is required by the CMS Waiver program. Developing useful process and outcome measures can also support ongoing quality improvement efforts. Drawing from the experience of Brigham Health’s Home Hospital program and others around the country, this webinar will examine a range of quality and safety measurement issues and how these data are essential to generating better outcomes and recruiting institutional funding and support.
